Insurance, rates, and billing

Crestview accepts most major plans, and self-pay options are available for everything we offer. Here's what to expect before your first visit.

Accepted plans

Insurance we accept

These are the plans the Crestview office currently accepts. Blue Cross Blue Shield is on its way, so if yours isn't listed yet, it's worth asking.

TRICARE: we see TRICARE patients as an out-of-network provider. TRICARE Select works directly. TRICARE Prime needs a referral, and you would be responsible for the balance until you reach your deductible.

Please note: Not all plans cover mental health services. Please check with your insurance provider for coverage and any out-of-pocket costs. The two offices accept nearly the same plans, but check the list for the office you'll be seen at.

Without insurance

Self-pay options available

If you're paying out of pocket, or your plan isn't one this office accepts, self-pay options are available for every service offered: initial psychiatric evaluations, follow-up medication management, and extended sessions.

Current rates are provided when you book, and before any charge is made. Ask at any point and we'll walk you through exactly what a visit will cost.

Superbills are available for out-of-network reimbursement. Ask and we'll provide the documentation your insurer needs to reimburse you directly.

Payment methods

We prefer to keep a card on file through our EHR, SimplePractice, and charge it when services are rendered.

If you're not comfortable keeping a card on file, we accept exact cash at the time of your visit.

Billing questions

Billing questions come straight to us. Call (850) 842-7835 and mention it's about billing.

Behind the scenes, Therassist, our billing service, helps us submit claims and resolve denials. It's also worth checking your own plan's mental health coverage before your first visit.
