For referring providers

Refer a patient

For primary care, therapists, hospitals, and community agencies sending a patient to SeaChange Mental Health. Two ways to reach us, and a short list of what helps us act on a referral the same day it arrives.

Two ways to refer

Send it by fax or by email

Preferred

Fax

(850) 955-3970

Attention: Referrals

Fax is the right channel for anything clinical. Send the referral form or a note along with whatever records are relevant, and you can include protected health information without the caveats that come with email.

Also fine

Email

info@seachangecare.com

Monitored during business hours

Ordinary email is not a secure channel. If you email, send the patient's name and a callback number and we will follow up for the rest, or ask us for a secure route. Please do not put a full clinical history in an unencrypted message.

Questions about whether a patient is a fit? Call (850) 842-7835. That number reaches both offices.

What to send

What helps us act on it

None of this is mandatory. A name and a phone number is enough to start. Everything below simply saves a round of phone calls before the patient can be seen.

  • Patient name and date of birth
  • A phone number the patient answers, and whether we may leave a voicemail
  • Insurance: carrier and member ID, or a note that they intend to self-pay
  • Reason for referral: working diagnosis, presenting concerns, what you are hoping we take on
  • Current medications and doses, including what has been tried and stopped
  • Relevant records: recent labs, prior psychiatric notes, discharge summary if they are coming out of a hospital
  • Any pharmacogenomic testing: if the patient has had GeneSight or a similar panel, send the report. If they have not and you think it would help, say so and we will discuss it with them
  • Which office, or that telehealth is fine
  • Urgency, if it is anything other than routine

What happens next

  1. We contact the patient

Our office usually reaches out to the patient within 24–48 hours to schedule. Nothing is booked online at this practice, so a person handles every referral.

  2. Intake and insurance

Consent forms, history, and benefits are handled through SimplePractice before the first visit.

  3. Evaluation

A 60-minute psychiatric evaluation, in person or by secure video, then follow-up at whatever interval the plan calls for.

Will you hear back? If we can't accept the patient's insurance, we fax you so you can redirect the referral. Otherwise we don't send a routine note back after the evaluation.

Scope

Who we can see

Navarre Office

Faiza Greenway, PMHNP-BC

7552 Navarre Pkwy Unit 41, Navarre, FL 32566

Psychiatric evaluation, medication management, and psychotherapy. In person, or telehealth anywhere in Florida. GeneSight pharmacogenomic testing is available where it would inform prescribing.

Ages 12 and older. Younger children, from around age 10, may be seen mainly for ADHD. Call to ask.

Accepted insurance at Navarre →

Crestview Office

Jasmine Lewis, PMHNP-BC

160 Alabama St, Crestview, FL 32536, inside Mindfully You Therapy Services

Psychiatric evaluation, medication management, and psychotherapy. Telehealth anywhere in Florida now; in-office visits in Crestview expected from November 2026. Adults 18 and older.

Accepted insurance at Crestview →

Accepted plans differ between the two offices, so check the office you are referring to rather than assuming a practice-wide list. Both providers are licensed in Florida, and telehealth patients must be physically located in Florida at the time of the visit.

Do not use this route for an emergency

Faxes and email are not monitored around the clock, and a referral is not a same-day disposition. If your patient is in acute crisis, direct them to 988 or 911, or to the nearest emergency department. Local crisis options are listed on our crisis resources page.

Are you the patient, not the referrer?

You do not need a referral to be seen here. Send an appointment request and our office will contact you to schedule.
