The form runs in SimplePractice, the records system both offices use. Its web address carries Faiza Greenway's name because it was set up first; Crestview requests are welcome through it and go to Jasmine.

Before your first visit
One form for both offices. Tell us which office, whether you want to be seen in person or by video, and roughly when you're free. We reply with times. Nothing is auto-booked, so a person has read your request before anything is confirmed.
Consent forms, history, and insurance details are handled through SimplePractice before your visit, so your appointment is spent talking.
In person or by secure video. You'll leave with a plan you agreed to.
We ask for 24 hours' notice to cancel or reschedule. No-shows and late cancellations may be charged a $50–$75 fee. This applies at both offices.
