Terms & Conditions

Effective September 22, 2026

1. Acceptance of these terms

By using seachangecare.com, you agree to these terms. If you don't agree, please don't use the site.

2. Who we are

This site is operated by SeaChange Mental Health LLC, doing business as SeaChange Mental Health, with offices in Navarre and Crestview, Florida.

3. Medical disclaimer

This website does not provide medical advice. Its content is general information only, and reading it does not create a provider–patient relationship. A provider–patient relationship begins only when you become an established patient of the practice through our intake process.

Never disregard professional medical advice, or delay seeking it, because of something you read here.

This website is not monitored for emergencies. If you are experiencing a mental health emergency, call or text 988, or dial 911. See our crisis resources page.

4. Requesting an appointment

This website does not book appointments. Every scheduling link takes you to a secure request form hosted by SimplePractice. Submitting that form is a request only: it does not create, confirm, or hold an appointment. You have an appointment once our office confirms a specific time with you.

Both the Navarre and Crestview offices use the same request form, and requests for either office are received in one SimplePractice account. Say which office you want so your request reaches the right provider. Requests are not monitored around the clock and are not a way to reach us urgently.

Intake paperwork and secure messaging also take place on SimplePractice. Your use of that platform is governed by its own terms and by the practice's patient agreements, not by these website terms.

5. Telehealth

You must be physically located in a state where your provider is licensed at the time of your appointment. Licensure follows where you are during the visit, not where you live. If you travel out of state, we may need to reschedule.

Telehealth has real limits. A video visit cannot include a physical examination, laboratory work, or any hands-on assessment, and it is not a substitute for emergency care. Where an in-person evaluation, labs, or urgent care is clinically necessary, we will tell you and help you arrange it locally.

You are responsible for having a device with a working camera and microphone, an adequate internet connection, and a private space for the visit. Technology can fail. If a connection drops and cannot be restored, we will attempt to reach you by phone and, where appropriate, complete or reschedule the visit at no additional charge.

Certain medications, including some controlled substances, carry additional requirements under state and federal law and may require an in-person visit or extra monitoring before they can be prescribed.

Informed consent for telehealth is obtained as part of your intake paperwork before your first video visit. You may withdraw that consent and request in-person care at any time.

6. Appointments, cancellations, and fees

We require 24 hours' notice to cancel or reschedule an appointment. No-shows and late cancellations may be charged a $50–$75 fee. This policy applies at both the Navarre and Crestview offices.

Current rates are published on each office's insurance page: Navarre and Crestview.

7. Insurance and payment

Accepted plans differ between our two offices. Check the insurance page for the office you'll be seen at. A carrier listed for one office is not necessarily accepted at the other.

Carrier participation and coverage can change. Listing a carrier on this site is not a guarantee of coverage or of benefits under your specific plan. You are responsible for confirming your benefits with your insurer and for amounts your plan does not cover.

8. Intellectual property

The SeaChange Mental Health name, logo, site content, and photographs are owned by the practice or used with permission, and may not be reproduced without written consent.

9. Third-party links

This site links to outside organizations, including crisis lines, Psychology Today, and our patient portal. We don't control those sites and aren't responsible for their content, availability, or privacy practices.

10. Disclaimer of warranties & limitation of liability

This website is provided “as is.” We work to keep it accurate and available, but we make no warranty that it will be uninterrupted, error-free, or current at every moment, and we disclaim implied warranties of merchantability and fitness for a particular purpose to the fullest extent Florida law allows.

To the fullest extent permitted by law, SeaChange Mental Health LLC is not liable for indirect, incidental, or consequential damages arising from your use of this website.

Nothing in this section limits our responsibility for the clinical care we provide. Professional liability for treatment is governed by Florida law and by your patient agreement, not by these website terms, and nothing here waives any right you have as a patient.

11. Accessibility

We aim to keep this site usable for everyone. If you encounter a barrier, please contact us at info@seachangecare.com and we'll work to fix it.

12. Changes to these terms

We may update these terms. Continued use of the site after an update means you accept the revised terms.

13. Governing law

These terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Venue lies in Santa Rosa County, Florida, where the practice's principal office is located.
