In her words
A Florida native, I'm a wife, mom, and proud grandmother. When I'm not working, I enjoy spending time with my family and my fur babies, including Murphy, my Sheepadoodle, who keeps life interesting! I also love traveling, exploring new places, and making memories with the people I love. Family, connection, and enjoying life's meaningful moments are incredibly important to me.
With 26 years of nursing experience, I am dedicated to providing compassionate, evidence-based mental health care in a supportive, judgment-free environment. As a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC), I provide psychiatric evaluation, medication management, and personalized treatment for adults ages 18 and older.
I believe that the best outcomes come from working together. Through shared decision-making, I partner with each patient to create an individualized treatment plan that considers the whole person: mind, body, lifestyle, and personal goals. My approach is holistic, collaborative, and focused on helping you feel heard, understood, and empowered throughout your care.
I offer both secure telehealth and in-person appointments to make accessing care as convenient as possible. Evening and weekend appointments are available as needed to accommodate busy schedules.
Whether you are seeking support for anxiety, depression, ADHD, mood disorders, or other mental health concerns, I strive to provide a welcoming space where you can feel comfortable, respected, and confident in your treatment journey.

Clinical background
Jasmine Lewis, MSN, APRN, PMHNP-BC, is a board-certified Psychiatric Mental Health Nurse Practitioner with more than 26 years of nursing experience across a variety of clinical settings. Her extensive nursing background has provided her with a broad understanding of the connection between physical health, emotional well-being, life experiences, and mental health.
Jasmine earned her Master of Science in Nursing with a focus in Nursing Leadership and Management before completing advanced graduate education as a Psychiatric Mental Health Nurse Practitioner. Her clinical experience includes community-based outpatient mental health, and telehealth where she has worked with adults experiencing depression, anxiety, ADHD, mood disorders, trauma-related concerns, and other behavioral health conditions.
She has a particular passion for women's mental health and recognizes that emotional well-being can be influenced by the many transitions and challenges women experience throughout adulthood. Her approach emphasizes thoughtful psychiatric assessment, evidence-based treatment, medication management when appropriate, and shared decision-making.
Drawing upon more than two decades of nursing experience, Jasmine strives to provide care that looks beyond symptoms alone. She values listening carefully to each patient's concerns, understanding the whole person, and developing individualized treatment plans that support meaningful and sustainable improvements in mental health and quality of life.
Telehealth across Florida now, and in person in Okaloosa County from November 2026.
