Jasmine is a board-certified Psychiatric Mental Health Nurse Practitioner with 26 years of nursing experience, providing psychiatric evaluation, medication management, and personalized treatment for adults ages 18 and older.
Her approach is holistic and collaborative. Through shared decision-making, she builds a treatment plan around the whole person: mind, body, lifestyle, and personal goals, all in a supportive, judgment-free environment.
Board certified as a PMHNP-BC and licensed in Florida, so she can see you by secure video anywhere in the state. In-office visits in Crestview are expected to start in November 2026.

TRICARE: we see TRICARE patients as an out-of-network provider. TRICARE Select works directly. TRICARE Prime needs a referral, and you would be responsible for the balance until you reach your deductible.

In-office visits are expected to start in November 2026. Until then, Jasmine sees patients by secure video.
By appointment only. There's no front desk or walk-in reception at this office. Every visit is scheduled in advance, and you'll be let in at your appointment time. Evening and weekend times are available as needed.
